### Exciting Opportunity for a Hospital Social Worker! **Job Description:** - Develop post-acute care plans for patients facing personal, social, or environmental barriers to wellness. - Collaborate with Case Manager RN to create effective recovery strategies. - Address complex psychosocial needs and assist with eligibility determinations for social programs. - Provide crisis intervention for patients and families experiencing psychosocial challenges. - Facilitate discharge planning for high-risk patient populations. - Communicate effectively with patients and families to ensure comprehensive continuing care plans. - Resolve personal and environmental difficulties affecting health and medical care outcomes. - Screen, assess, and provide resources related to social drivers of health for patients and families. - Act as an expert on available community resources for social drivers of health. - Refer patients to Behavioral Health for assessments and outpatient services as needed. - Educate and support families regarding food access and housing solutions. - Coordinate patient transportation programs for smooth transitions. - Conduct financial assessments and facilitate referral services. - Arrange for in-home care and necessary medical equipment. **Who You Are:** - Compassionate, empathetic, and understanding professional. - Highly motivated, creative, and resourceful individual. - Exceptional communicator with strong interpersonal skills. - Proactive problem identifier and effective problem solver. **What You'll Need:** - Bachelor’s degree in Social Work required; Master’s degree preferred. - California Licensed Clinical Social Worker (LCSW) required. - 2 to 5 years of hospital social work experience preferred. **Shift Information:** - Full-time, day shifts. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
